Sarah C. DeSilvey, MSN, FNP-C, is a rural family nurse practitioner. She balances clinical practice with quality consulting and regional and national advocacy for addressing the social determinants of health. She earned her Masters at the University of Vermont where her thesis focused on the outcomes of the first implementation of food insecurity screening in an inpatient pediatric setting. She is a regional clinical representative to Vermont’s innovative Accountable Care Organization, OneCare Vermont. In this role she represents her health service area in health reform and quality initiatives and spearheads programs focused on social health integration, and social determinant risk calculation. She sits on both the Clinical and Quality Advisory Board and Pediatric Subcommittees for OneCare Vermont and the Board Quality and Safety Committee and Quality Improvement Committee for her hospital, Northwestern Medical Center. She has a particular interest in developing the language and terminology to capture the process of caring. She is a national expert on social determinant coding and terminology. This work led her to purse a Doctorate of Nursing Practice at Yale University. Her doctoral project aims to apply standards of ethics and evidence to the care of food insecure populations through development of terminology and diagnostic criteria. The goal is to improve the care for food insecure patients, across settings. Sarah is current pediatric faculty at the Larner College of Medicine at the University of Vermont.
