Biography

As a critical care nurse working in the hospital setting, I have 32 years of experience in caring for the acutely ill populations including patents with congestive heart failure (CHF). My areas of interest are in cardiology, self-care and quality of living for our patients and families. In my clinical nurse specialist (CNS) role, we are seeking to reduce our 30-day readmission rates for CHF patients. To accomplish this, we must look at improved patient education and improved communication modalities. Motivational interviewing and the use of home tele monitoring can be an adjunct to improving self-care and self-management of this chronic disease management process. Patient perceptions and adherence with home tele monitoring is also of interest. There is not cure for CHF. Therefore, I am also interested in the transition for acute disease management to the palliative care focus, while maximizing quality of living. I have been recognized in the San Diego community as an outstanding Advanced Practice Nurse (2013 award). The PhD education, which I am obtaining at the University of San Diego, is providing me with the foundational knowledge in how to improve my knowledge and skills as a nurse scientist. In my CNS role, I am leading groups of interprofssionals on several committees and councils. I plan to use my newly obtained knowledge to strengthen my leadership skills and continue my pursuit of various forms of nursing research

My nursing career began in 1986 as I pursued an LPN degree. I graduated in 1988 and briefly worked as a charge nurse in a nursing home. I knew there was more for me and desired to continue my education. I moved to Oklahoma and returned to school. I completed my BSN in 1991 and began working in a Neonatal Intensive Care Unit. I had incredible medical and nursing mentors and soon learned that neonatal nursing was my passion. In 1995 I joined the USAF. My 19 year career has all been spent in the neonatal specialty between three military treatment facilities: Wilford Hall Medical Center, Lackland AFB, TX-7 ½ years, US Naval Hospital Okinawa, Japan-10 ½ years, and Langley AF Hospital, Langley, VA since July 2103. During my AD career, I served as an Extracorporeal Membrane Oxygenation (ECMO) specialist and nurse coordinator for the only DoD program. I served as transport coordinator, grief counselor, out reach educator, preceptor and charge nurse. I taught a myriad of neonatal courses, contributed to orientation programs and spoke at local and national neonatal conferences. I served as Nurse Manager, Department Head and Clinical Nurse Expert. My MSN, NNP and now my DNP studies are as a civilian student as I continue my active duty responsibilities. I am eligible and plan to retire November 2016. I am a single mother of three boys: Martel 29 years old, Tavarus, 28 years old and Dennis 13 years old. I have a 4 year old grandson, Hudson.

I have been a Registered Nurse for five years with a background in acute, cardiac care. Currently, I work as a Clinical Nurse II at UCSF Medical Center. I work on the adult cardiothoracic, vascular, and transplant transitional care unit at the Parnassus Heights campus. A large portion of the patients I care for have been diagnosed with advanced stage heart and/or lung disease, admitted for either exacerbations of chronic conditions or evaluations and implementation of advanced, high-risk therapies. Through my experience in the acute care setting, and particularly working with this specialty population, I have developed a strong interest in how we as a healthcare system may improve primary and secondary prevention services.

Dissertation: I am finishing my first year in the DNP-FNP/MPH program at University of San Francisco, expecting to graduate in 2021. Though some conditions are genetic, a number of contributing factors to disease development are rooted in lifestyle choices and environmental exposures. For older adults, especially those from lower socioeconomic backgrounds, it is possible that individuals may have experienced a lifetime of harmful environmental exposures through previous working and living conditions. With this in mind, my goal is to develop an environmental hazard screening tool for the outpatient, community setting bundled with a targeted adult and older adult education program. The program would be aimed at identifying and reducing exposure to controllable environmental risk-factors so as to prevent complex health concerns from developing or further progressing. With a significant proportion of the general population entering older adulthood, it is imperative that we as a healthcare community provide the necessary education and resources to support healthy aging in a meaningful and accessible manner.

Maria Thompson is a psychiatric nurse practitioner at the VA Boston Healthcare System, Jamaica Plain, General Mental Health Outpatient Clinic caring for veterans with diverse mental health illnesses. She completed a one-year VA Mental Health Nurse Practitioner Residency program in 2014 at the Ronald H. Johnson VA in Charleston, South Carolina. In 2014, Maria trained as a facilitator at American Psychiatric Nurses Association’s Recovery to Practice Program. She obtained her Master’s in Mental Health Nursing in 2009 from State University of New York at Stony Brook and has served as a preceptor for nursing students and nurses. In 2009, Maria earned the Nurse of Excellence award from Long Beach Medical Center. In 2007, she was a candidate for Bridging the Gap: Linking Clinical Practice to Academia Nurse Preceptor/Faculty Training at the Greater NY Hospital Association. I am a nurse informatician passionate about discovering data-driven, information technology solutions for increased patient and provider engagement and incorporating them into interdisciplinary clinical practice. With over 20 years of nursing experience at New York Presbyterian Hospital, I have continued to move to more responsible positions into my current role as a nurse scientist. I have over 10 years of experience of giving clinical input to technology projects in all areas, especially regarding the implementation of an electronic medical record and patient facing tools. I am a resource for projects that involve clinical workflow optimization and assist in finding solutions for difficult clinical informatics challenges. As the nursing lead for the design, implementation and roll-out of an institution-developed personal health record (PHR), myNYP.org, I evaluate the patient use of health IT tools and seek out ways to bring patient generated data to the clinical environment. Furthermore, I coordinate research efforts to understand the clinical implications related to the sharing of data between patients and clinicians as well as the benefits to the learning health system. I serve on the steering committee for the Alliance for Nursing Informatics and recently completed a fellowship in the ANI Emerging Leaders Program assessing nurse readiness to use health IT tools for patient engagement. I completed a MSN in Nursing Informatics from Columbia University, BSN from the University of Virginia and I’m currently pursuing a PhD from the University of Utah with a focus on the integration of mobile health data into clinical workflows.

Ellen Tilden RN, MS, CNM earned a Bachelor of Arts in Political Theory from Reed College and a Master of Science and Certificate in Nurse-Midwifery from the University of California, San Francisco. She has practiced as a full-scope CNM since 2000. Ms. Tilden is currently in the second year of her PhD training in the School of Nursing at Oregon Health and Science University, Portland Oregon. She is passionate about identifying risk-appropriate care for healthy pregnant and laboring women for the purpose of reducing unnecessary or harmful interventions during labor, including decreasing primary cesarean delivery rates. Her doctoral research seeks to evaluate if healthy pregnant women’s participation in group prenatal care is associated with higher rates of active labor (versus latent labor) hospital admission and lower rates of cesarean delivery without evidence of harm to the neonate. In 2014, she was published in the Obstetrical and Gynecological Survey as the first author of ‘Group Prenatal Care: Review of Outcomes and Recommendations for Model Implementation.’ Ms. Tilden has received several scholarships and awards during her doctoral training including the Sigma Theta Tau Beta Psi Award, the American College of Nurse-Midwives (ACNM) W. Newton Long Award, and both the ACNM 2014 Best Poster Award and the ACNM Division of Research 2014 Best Research Poster Award. She is a member of Phi Beta Kappa and Sigma Theta Tau honor societies.

Inquisitive by nature, I am committed to life-long learning and the application of that knowledge in the service of others. Prior to my current career as an Educational Nurse Coordinator, I worked in a steel mill for twenty-seven years. While working at the steel mill, I also attended college classes and pursued a part-time military career in the Michigan Army National Guard as a Military Police Officer. Presently, I am a Lieutenant Colonel in the United States Army Reserve and support Battalion Staffs as an Observer/Controller-Trainer. Initially, I earned an Associate Degree in Nursing and was awarded a Honor’s Scholarship. I then continued my studies at Wayne State University and earned a Presidential Scholarship. I completed a Bachelor Degree in Psychology and then continued my nursing studies in an accelerated BSN-MSN program. In 2003, I began my nursing career with the University of Michigan Healthcare System. While working as an Operating Room Registered Nurse, I completed my Acute Care Nurse Practitioner M.S. degree and then post-graduate studies for certification as a Nurse Educator. As a student in the University of Michigan Doctorate of Nursing Practice (DNP) Program, I will exploit my opportunities as a Jonas Veterans Healthcare Scholar and apply transformational leadership skills in the realm of nursing. I am learning new skills in implementation science and how to conduct sound, scientific research. I have a hunger to: effectively implement these skills, enhance patient care and workplace safety, and serve as a nursing leader and scientist.

Biography

MJ Tran, MBA, BSN is a PhD student at the University of Utah College of Nursing. She has over nine years of experience in healthcare innovation, care management and transitions of care across the lifespan. In her current role, she envisioned a robust community-based Transitional Care Management program that reduced re-hospitalization rates by 50% by establishing communication links in healthcare settings that did not previously exist. Ms. Tran’s overarching goal is to improve patient outcomes, quality and cost-effectiveness of care for older adults and veterans with multifaceted conditions. Her dissertation will explore the role of payer standard authorization protocols on inappropriate skilled nursing facility admission denials for older adults and veterans transitioning from the home setting to a higher level of care. Due to lack of caregivers, social and supportive care limitations, as well as high-risk complications, this growing population continues to face vulnerability in seeking adequate quality of life and care. This project aims to avoid costly hospitalizations by identifying lower cost and appropriate solutions for vulnerable populations with multimorbidity. Ms. Tran completed her BS in Biology from Utah State University, her BSN from Creighton University, an MBA from Westminster College, and is the recipient of the 2017 Utah Healthcare Hero Award for Community Outreach. She is a firm believer that there should be no trade secrets in healthcare and advocates to challenge the status quo of standardized care. In working collaboratively rather than as individuals, the healthcare community can be rebuilt to empower the most vulnerable populations.

Dissertation: Exploring the Impact of Post-Acute Admission Denials by Medicare Advantage Payers for Homebound Older Adults and Veterans with Multimorbidity In transitioning homebound older adults with physical and psychiatric disorders to a higher level of care, many barriers were discovered in the standard authorization guidelines for Medicare Advantage payers. These barriers resulted in care delays, denials, increase in hospitalizations and readmission, diminished qualify of life, as well as fatalities. The purpose of this dissertation is to explore the long-term impacts on patient outcomes and costs to the healthcare systems due to standardized protocols. The overarching goal of this dissertation is to bridge a care model focused on maximizing comfort and minimizing costs by identifying levels of care suitable for optimal outcomes.

Jasmine Travers is a second year pre-doctoral student at Columbia University School of Nursing Center for Health Policy. Prior to Columbia, she earned her BSN from Adelphi University, MSN from Stony Brook University and holds her board certification in Adult Gerontological Health by the American Academy of Nurse Practitioners. Her research interest lies in health disparities among minority populations and nursing workforce diversity. Ms. Travers worked as a critical care staff nurse, and clinical nurse educator. Currently, she serves as an Adjunct Faculty member for the School of Nursing and Public Health at Adelphi University and plans to continue in academia as a nurse scientist and educator upon completion of her degree. For the 2014-2016 years, Ms. Travers has received funding support from a NINR NIH Diversity Research Supplement aimed to increase diversity in health-related research. Under this supplement and the guidance of her mentoring team, Ms. Travers will examine relationships between diversity in sociodemographics amongst nursing attendants and the provision of quality nursing home care as well as health disparities in long-term care setting residents related to infection control structures and processes in preventing HAIs in nursing homes. At the start of the 2014 academic year, Ms. Travers will also serve as the student representative to Columbia University’s School of Nursing PhD Faculty Committee.
